The phrase "SAFE, SANE, and CONSENSUAL" is often quoted in the D/s & BDSM community. An ACTIVITY and INTEREST survey is a list of activities that a servant/submissive can use to help clarify her needs and assure exciting, fulfilling, interaction with her Master/Dom.
It's unlikely you will find a Master who can read your mind, nor a Dom equipped with a Photographic memory, so an actual list will help prevent misunderstandings that can cause emotional or physical harm.
The Survey also helps an inexperienced or naive submissive to consider new and exciting activities she may not have thought about. Partners may re-visit the checklist and revise it as they grow together in trust and experience. The survey can be used in combination with a signed contract, if both parties feel it's better to have something more formal.
